We are looking for an enthusiastic, knowledgeable, and compassionate individual to join our Carer Team, playing a vital role in supporting unpaid carers across the community.

This rewarding role focuses on identifying, supporting, and empowering carers, ensuring they have access to the right information, services, and support to maintain their wellbeing and independence.

About the Role

  • Work directly with unpaid carers, including those who may be hidden or hard to reach
  • Provide tailored practical, emotional, and advisory support, including assessments, reviews, and support planning in line with statutory duties
  • Offer guidance on services, benefits, and carers’ rights, helping individuals navigate complex systems
  • Build strong partnerships with health, social care, education, and voluntary organisations to deliver coordinated support within neighbourhoods to young carers and young adult carers (up to 25), helping them build independence, resilience, and life skills
  • Lead on smooth transitions from children’s to adult services, advocating for young adult carers within education, health, and social care systems
