Weekly Communication – Week Commencing 23/10/2023

Dear Colleagues

Please see your weekly overview update on behalf of the Director of Adult and Community Wellbeing (DASS) and Director of Public Health (DPH).

 Discharge Facilitation Form

The Integrated Discharge Team are currently piloting the attached form at the top of this email.  This will be sent to you rather than the D2A form.  As you can see, the form contains more information but please review this yourself and if any other information is required, let me know and I will add to form.
The form will be completed by members of the Integrated Discharge Team.  They will be going onto the wards to speak to the person, their families and the ward staff to gather relevant and correct information.  It is envisaged that this will support a safe and appropriate discharge from hospital.
Any feedback on the form would be appreciated.
